Trip Reservation Information

To secure a place for a trip, please submit a deposit by check made out to Belmont Hill School along with an electronically completed and hand-signed Trip Reservation Form (linked in box below) to Cheryl Wolf in the Main Office; deposit amounts and due dates are listed on the Group Trips webpage. Please download the form first on to your desktop. Complete the form BY DIRECTLY TYPING ONTO THE FORM, FILLING IN ALL BOXES WITH THE INFORMATION REQUESTED. Then, PRINT OUT THE FORM AND SIGN BY HAND THE SIGNATURE SECTIONS.
